Paid Time Off (PTO) Request
Use this form to request Paid Time Off (PTO) for vacation, personal time, or other non-sick leave. Submitting the form helps ensure your time off is reviewed properly and recorded for payroll.
Please note: Submitting this form does not guarantee approval. Time off is not confirmed until it has been approved by your manager and verified by HR/payroll.

• Available only to full-time employees working 30+ hours/week (1,500 hours/year).
• PTO becomes available after your first work anniversary.
• Time is accrued throughout the year based on the hours you work:
○ Year 1: Up to 40 hours earned (.01944 hrs per hour worked)
○ Year 2+: Up to 80 hours earned (.03854 hrs per hour worked)
• PTO resets on your anniversary date and does not roll over year to year.
• Unused PTO is forfeited when employment ends (voluntary or not).
• PTO is paid at minimum wage for tipped employees.
• If you’re paid at multiple rates, a weighted average is used.
• PTO is paid on regular payroll (max 8 hrs/day).
• You can use PTO on holidays if your work location is closed.
• You must submit a PTO Request Form for approval. PTO will not be paid until your request is confirmed by your manager and HR/payroll.

Paid Sick Time (PST) Request
Use this form to request Paid Sick Time (PST) if you're unable to work due to illness, injury, or another health-related reason. You’ll be asked to include the dates and hours requested, along with a general reason for use.
Please note: Submitting this form does not guarantee approval. Sick time is not confirmed until it has been approved by your manager and verified by HR/payroll.

• Available to all employees, regardless of full-time or part-time status.
• PST begins accruing as of May 1st, 2025, or your hire date (whichever is later), and is available to use as it accrues.
• Time is accrued based on the hours you work:
○ 1 hour of PST earned for every 30 hours worked
○ You may use a maximum of 56 hours of PST per year
○ Up to 80 unused PST hours may be rolled over year to year.
• Unused PST is forfeited when employment ends (voluntary or not).
• PST can be used for:
○ Your own illness or injury
○ Caring for a sick or injured family member
○ Preventative care or doctor’s appointments
○ Public health emergencies (e.g., closures or quarantine)
○ Certain situations involving domestic violence, assault, or stalking
• PST is paid at minimum wage for tipped employees.
• If you’re paid at multiple rates, a weighted average is used.
• PST is paid on regular payroll (max 8 hrs/day).
• If you are requesting to use PST for more than 3 consecutive days, you may be asked to provide a doctor’s note or personal statement. We will never request a diagnosis or confidential medical details.
• If your sick time is planned (like a doctor’s visit), please give your manager as much notice as possible. For emergencies or unplanned absences, notify your manager as soon as you can.
• You must submit a PST Request Form for approval. PST will not be paid until your request is confirmed by your manager and HR/payroll.
